Transitional Coordinator
Transitional Coordinator Step 9 ($32,936) – Step 29 ($59,486) Full-Time Benefits Regular – Full –Time – Grant Funded This position is a grant funded position with a limited duration. This position will work directly within Tribal Community implementing objectives of Klamath Tribes IMPACTS grant. This position will be responsible for implementing jail diversion strategies including brief interventions, arraignment advocacy, and connection to behavioral health resources. This position will use a community based, trauma-informed, and equitable approach in all areas of engagement. This position will work collaboratively with varying institutions and judiciary systems. This position is highly mobile and will require individual to maintain stringent daily schedule, perform work both remotely and in-office, utilize electronic health records, exercise strong communication consistently. Klamath Tribal Health & Family Services is a division of the Klamath Tribes and is responsible for providing healthcare services to the Native American population who reside in Klamath County. Any person living in Klamath County who is able to provide proof of their Native American heritage is eligible to receive healthcare services at The Klamath Tribes Wellness Center.
