Step Range: 15 ($41,686) – 34 ($73,098) Full-Time Benefits Regular – Full -Time

The primary objective is to identify available support services in Klamath County and/or through The Klamath Tribes. The Case Manager will build relationships with participants and provide information on utilization requirements for receiving identified services. The Transitional Emergency Shelter Case Manager will work with participant to provide intensive case management services in collaboration with KTHFS programs such as medical clinic, dental clinic and behavioral health clinic. The TES Case Manager will identify the potential need for additional services for Klamath Tribal Health & Family Services (KTHFS) eligible participants on a case by case basis and will provide assistance, to those identified, with access and coordination of appropriate services. The TES Case Manager must have the ability to work with participants in distress and/or crisis. This position will work directly with the at-risk, homeless population and must be skilled in de-escalation and mental health support techniques.
